Admissions Checklist

• Application Form

• Medical Form (including Emergency Contact information)
  Application Fee (Nafl. 100 or $55 per student)

• Cover letter written by Parent or Legal Guardian requesting admission
  Copy of Passport or ID
  Immunization Records
  School Records (current year and previous 2 years for students grade 2 and higher)

• One (1)Teacher Recommendation Form (for all students grades Pre-K - 8)

• Short autobiography written without assistance (for students going into grade 3 through 5)

• Applicant Questionnaire (students going into grade 6 and higher

• Two (2) Teacher Recommendation Forms (preferably from English and Math teachers)
• Native English speakers should submit 2 page writing sample, either creative or expository
